Membership Form - Required Information

1. Full Name: Used mainly for communication. For example, we’d like to address you by your name in emails if you need technical support.

2. Email Address: This gives your account a unique identifier and is used mainly for technical support communication (e.g. sending you a form to re-set your password if you forget it). For teacher accounts, you can choose to opt into our email list. You can unsubscribe from this list at any time by following the link at the bottom of any email. Student accounts are not allowed to subscribe to our email list (even if they request to do so on the sign up form).

3. Password: Used to keep your account secure. All passwords are hashed and stored in a protected external server. We don’t have knowledge of your password.

4. I’m here for…: Used mainly to give Skew The Script information on the number of users who are making use of our different curricula and projects.

Membership Form - Additional Information Collected from Teachers

Note: Students do not provide this information to register.

1. School Email: Used mainly to verify that users creating teacher accounts are, in fact, teachers at educational institutions. It’s also used as “backup” contact information teachers, if they have technical issues and cannot be reached at their primary email, and to gauge our impact and exposure in different schools/areas.

2. Name of School: Providing this information helps us gauge our impact and exposure in different schools.

3. Location (City and State of School): Providing this information helps us gauge our impact and exposure in different areas.

4. School ID Number: This information links member records with publicly available school listings (e.g. the NCES database). It’s used to verify that the institutions listed by new members are, in fact, accredited educational institutions. This information also helps us gauge our impact and exposure across schools and demographics.

5. Number of Students: New teacher accounts provide an estimate for the number of students they will be using our materials with, in the current school year. This information helps us gauge the exposure and impact of our materials.

Optional information collected from both teacher and student sign-ups: “How did you hear about us?” Providing this information helps us gauge how new users tend to find our site.
